We have updated our Japan Sports Center Application Form for all members.  Copies of this new form can be picked up at the dojo the next time you are in, please fill the new form.

Please note, there are a couple of new sections where you will need initials.

Can this new updated form please be filled out and returned as soon as possible to Hanshi or Victoria?
